The Australian Association of Consultant Pharmacy Award recognises outstanding pharmacists who work collaboratively with patients and medical practitioners to ensure safe medicine use in the home.
UQ School of Pharmacy lecturer Dr Freeman said recipients were selected from more than 2500 consultant pharmacists nationwide and nominated based on innovative practice, exemplary service and improved patient outcomes.
"It's a great honour to receive this award for work that is professionally satisfying and has a real impact on patient health," he said.
"Ensuring patients understand and use their medications safely and effectively can improve their quality of life and deliver optimum health outcomes."
Innovative consultant pharmacist
Dr Freeman is a former Pharmaceutical Society of Australia Young Pharmacist of the Year and is acknowledged as an innovative consultant pharmacist by his peers.
Consultant pharmacists play a pivotal role in liaising with patients, their doctors and allied health professionals to ensure medications are used appropriately.
Dr Freeman said his current role incorporated a consultant pharmacy service within a general practice medical centre.
"I've been able to provide a comprehensive and integrated service for patients by working closely with other practitioners," he said.
"Patients who have complicated medical conditions and multiple medications are referred to me for review by their general practitioner.
"I visit the patients in their homes to discuss their medications and recommend adjustments where required.
"This ensures patients receive the best quality treatment and ongoing care."
